City Health Practice - Bransholme

Bransholme Health Centre

Conforming to the Accessible Information Standard became a requirement of all GP practices in England on 31st July 2016.

It has been put in place to ensure that people who have a disability, impairment or sensory loss are able to access and understand any information or communication support they need.

We aim is to make sure that our patients and service users, and their carers and parents can access and understand the information they are given.  This will include making sure that information is available in different formats if needed for example, large print, easy read or via email. The practice will also make sure that patients and service users receive help and support with any communication need they have.

We will:-

Ask people if they have any information or communication needs and find out how to meet their needs.  New patients registering at the practice will have the option to specify a preferred method of communication.

Record the preference of their needs clearly

Highlight the patient's notes so it is clear they have information or communication needs.

Share information and communication needs with other providers of NHS and adult social care when there is a explicent consent to do so.

Take steps to ensure that people receive information that they can access and understand, and receive communication support if they need it.
